New Student Enrollment Application:
Click on the following link and complete the form on a computer or chromebook (currently it won't work on a cell phone or on the mobile device app). This will allow you to create an account on our online portal to begin the registration process: TeacherEase Registration Portal
Download and complete the following form so that we can request records from your student's previous school: Request for Records form. You may bring the form to the school when you are enrolling your student, or you may email the completed form to the Elementary Office ([email protected]) to expedite the process of acquiring your student's records.
When enrolling your student, please bring the following documents with you: your student's birth certificate, social security card, proof of residency (mortgage document, rental contract, or property tax statement), and immunization record. We will also need a copy of a parent/guardian's driver license or state id. If you have questions about anything in this process, please call the Elementary Office at 417-751-2534 (option 3).
